Silent Hill: Shattered Memories' ending pretty much did it for me. Shocked me and left me staring at the screen piecing everything together even as the credits were rolling.

In BioShock when Atlas turns out to actually be Fontain.  I kinda got the feeling that Atlas was hiding something but I never guessed that he was actually Fontain.  Come to think of it, BioShock was full of plot twists.

The unveiling of The Flood in Halo: Combat Evolved. Halo was a typical (albeit excellent) Sci-fi FPS, with aliens and epic space combat... but the sudden introduction of this parasitic zombie-virus and the creatures it creates took the game in a whole new direction. I certainly didn't expect it.
