In Pennsylvania, you need to read this pamplet, and then you have to go to the doctor and get a physical check-up, and then bring in the doctor's signature to certify that you have gotten your physical. Then you take the test, and if you pass, you get behind the wheel. After you pass the driving portion of the test, you get your Learner's Permit, and you need 50 hours total of driving. The only restriction is that, with the learner's permit, you must have someone with a driver's license with you. After you meet the requirements, then you get your permanent Driver's License.
